Data Application Form

APPLICATION FORM TO THE SENSITIVE DATA CONTROLLER

General Explanations

In the Law No. 6698 on the Protection of Personal Data ("KVKK"), personal data owners ("Applicant"), who are defined as the person concerned, are entitled to make certain requests regarding the processing of their personal data in accordance with Article 11 of the KVKK.

The application form can be submitted to Duyar Vana Makina San. Ve Tic. A.Ş. ("Duyar Valve"), to determine your personal data processed by Duyar Valve, if any, and to respond to your application accurately and within the legal period. In order to ensure the security of your personal data and to prevent unlawful data transfer, additional information may be requested by Duyar Valve for identification and authorization. In the event that the information specified by the Applicant is not correct and / or up-to-date or the requests are unauthorized, the Applicant is responsible for this matter.

Pursuant to Article 7 of the Communiqué on the Procedures and Principles of Application to the Data Controller, if the Applicant's application will be answered in writing, no fee is charged up to ten pages. A transaction fee of 1 Turkish Lira may be charged for each page over ten pages. If the response to the application is given in a recording medium such as CD, flash memory, a fee may be charged up to the cost of the recording medium.

Scope of the Right to Apply under Article 11 of the LPPD

The Applicant may apply to Duyar Valve and make a request on the following issues:

    1. Learn whether their personal data is being processed,
    2. Request information if their personal data has been processed,
    3. To learn the purpose of processing personal data and whether they are used in accordance with their purpose,
    4. To learn the third parties to whom personal data are transferred domestically or abroad,
    5. To request correction of personal data in case of incomplete or incorrect processing and to request notification of the transaction made within this scope to third parties to whom personal data is transferred,
    6. Although it has been processed in accordance with the provisions of the KVKK and other relevant laws, to request the deletion, destruction or anonymization of personal data in the event that the reasons requiring its processing disappear and to request notification of the transaction made within this scope to third parties to whom personal data is transferred,
    7. To object to the occurrence of a result against him/her by analyzing his/her processed data exclusively through automated systems,
    8. To request compensation for damages in case of damage due to unlawful processing of personal data.

Application Method

Pursuant to the first paragraph of Article 13 of the KVKK; applications to be made regarding these rights must be submitted to us in writing and signed or by other methods determined by the Personal Data Protection Board ("Board").

In this context, applications to be made in writing, by printing out this form;

  • By the Applicant's personal application by hand,
  • Through a notary,
  • Registered electronic mail (KEP) address, secure electronic signature, mobile signature or
  • The applicant has previously notified us and is registered in our system

Duyar Valve may request additional information and take necessary measures to verify the identity of the Applicant in applications to be made by the Applicant via e-mail.

In accordance with the second paragraph of Article 13 of the KVKK, your applications submitted to us will be responded in writing or electronically within thirty days from the date of notification or receipt of your request, depending on the nature of the request.
